Kindergarten/ New Student Registration Forms

All incoming Kindergarten students MUST have a completed Universal Health Record (completed within one year of the first day of school), Health History and up to date Immunizations in order to start school. If your child's birthdate falls after the first day of school, please provide written documentation of a scheduled doctor's appointment to have these completed. All NEW STUDENTS entering Hamilton Township school district are also required to have an updated Universal Health Record (physical) completed within the year of entry to school, Student Health History and updated immunizations.
