5th Grade Immunization Requirements

Students entering 6th grade are required to receive a Tdap (Tetanus, Diptheria, Acellular Petussis) and a Meningococcal-containing vaccine. These are expected to be completed soon after their 11th birthday. If your child has a birthday after the first day of school, please send in written documentation from the doctor with an appointment date of when these vaccines will be given. Please contact the school nurse with any questions or concerns.
